To begin the claiming process, please download your form using the link below. You should use this form to make your claim, making sure that you answer all of the questions as fully as possible.

The claim form clearly states what additional information and documentation is required in order to support your claim. We strongly recommend that you keep copies of all items that you send to us, just in case the originals are lost in the post.

Turnaround time for all claims is 10 working days upon first receiving your completed documentation. To enable your claim to be processed more efficiently and ensure the items do not get lost in the post, we recommend that you send your claim form to us as recorded delivery.
